Does My Mirror accept Chronic Disease Management Plan (CDM)?

Learn how to use a Chronic Disease Management (CDM) Plan to access Medicare rebates for psychology sessions with eligible AHPRA-registered psychologists through My Mirror.

If you have a Chronic Disease Management (CDM) Plan and a referral from your GP, you may be eligible for Medicare rebates on psychology sessions with eligible AHPRA-registered psychologists.

What is a Chronic Disease Management Plan?

A Chronic Disease Management Plan is designed to support people living with a chronic medical condition that has been present, or is expected to be present, for six months or longer.

To access services under a CDM Plan, your GP will need to prepare:

  • A GP Management Plan (GPMP)

  • Team Care Arrangements (TCA)

  • A referral to an eligible allied health provider

How do I use my CDM Plan with My Mirror?

1. Obtain a referral from your GP

Speak with your GP to determine whether you are eligible for a Chronic Disease Management Plan and a referral for psychology services.

2. Send us your referral

Email your CDM referral to [email protected].

3. We'll prepare your account

Once we receive your referral, our team will configure your account so eligible Medicare rebates can be applied to your psychology appointments.

How many psychology sessions does a CDM Plan cover?

A Chronic Disease Management Plan may provide access to up to five Medicare-subsidised psychology sessions per calendar year.

Please note that the Medicare rebate available under a CDM Plan is generally lower than the rebate available through a Mental Health Treatment Plan (MHTP/MHCP).

Can I claim Medicare rebates with a CDM Plan?

Yes. If you have a valid CDM referral and attend an eligible psychology appointment, you may be able to claim a Medicare rebate.
